Is there a bootPROM for either the FPGA or the FX2 or both? I note that
the Xylo has one. Otherwise, does the all the firmware have to be
downloaded through USB every time the board is turned on?


That's the EPCS1.

Also, since all but the power pins on the bus go only to the FPGA, the
line assignments on the left of the diagram are really "hints" as far as
the hardware is concerned; it's the firmware that really assigns the
meaning, right?

They are defined by the configuration file that is programmed into the FPGA. It's not really firmware.
